FINDINGS of the MIB

• CG XO:

  • Failed to properly analyze CVN movements.
  • Failed to take aggressive action to direct the OOD or assume the Deck/Conn and maneuver the ship out of an extremis situation.

• DDG:

  • Watch team on nearby DDG did not correlate the critical nature of the situation and provide warnings over available circuits.

• XO’s experience as a DCA on a conventional CV, and equally his lack of experience with nuclear CVN engineering operations, led to his failure to consider the possibility the CVN was backing as part of engineering plant control drills.

• The DDG’s involvement (or lack there of) was rejected as a significant cause.
