DURING OPEN OCEAN TRANSITS THE COMMAND HAD INSTITUTED A CDO U/W. IN PRACTICE, THE CDO RECEIVED ALL ROUTINE CALLS NORMALLY RECEIVED BY THE CO WRT MANEUVERING THE SHIP TO AVOID CONTACTS. WHILE THE CDO WATCH WAS NOT IN PLACE DURING THE MALACCA TRANSIT, IT WAS CONSIDERED A PROXIMATE CAUSE BY THE MIB BECAUSE THE OOD’S WERE NOT ACCUSTOMED TO CALLING THE CO WHEN REQUIRED.
