FINDINGS of the MIB
CVN CO:
- Failed to set a climate which fostered attention to detail in integrating shipwide evolutions
- Failed to ensure multiple events were integrated properly and failed to apply risk management
- Failed to fully establish a Combat Systems Department
Notes:
- The CVN CO was directly responsible for the capability of his departments to safely conduct the multiple missions of the CVN. In formation management, oversight, and integration of operational scheduling, to include add-on events among primary decision makers, was lacking on the night of the collision.
- Regardless of the appropriateness of conducting maneuvering transients during EMI testing, other alternatives for CG relative positon were available.
- The Combat Systems Department was established less than two months prior and had never had departmental responsibilities fully defined or formally promulgated.
