Notes:
- A more experienced or more senior OOD may have demanded better support from CIC and the bridge watch teams. Having the bridge watch in four sections diluted the watch bill and left the bridge watch reponsibilities to more junior people. The MIB believes a three section watch would have lessened the need for inexperienced watchstanders while still providing adequate crew rest.
- A more experienced TAO & SSWC would have ensured quality support for the bridge and would have been more proactive in communications restoral.
- CO’s trust in the XO was sufficient to allow the CO to leave the bridge even though his ship was having difficulty following the movements of the CVN.
